Instructions

This form should be used to appeal the loss of your financial aid due to poor academic performance, less than 67% credit completion due to withdrawal or leave from courses or semester(s), or both.

1. Review Simmons University's Undergraduate SAP Policy
2. Review the questions below and take some time to consider your answers. As this form cannot be saved, you should draft your response and return to this form when you are ready.
3. Collect the documentation you need to support your appeal and convert it to a digital format (MS Word or PDF).
4. Complete and submit this form by the deadline provided in our notification.

The thoughtfulness and quality of your appeal will factor into any decision made. Additionally, appeals submitted without the appropriate documentation will not be considered or reviewed. Carefully review your appeal before submitting it!

1. What is your understanding of the factors that contributed to your poor academic performance and/or being below the required 67% threshold of completed to attempted credits?
2. What has changed? What will you do differently? What are your specific plans to improve your academic performance and/or completion rate, achieve good standing, and reach SAP?

(If approved, these plans will become the basis of your official "Measurable Success Plan" (MSP). Your MSP will be reviewed at the end of each semester and must contain measurable goals that you can track in a MSP Log provided by the Undergraduate Advising Office.)

Before signing and submitting your appeal you must read and agree to each of the following.
I understand:

I understand: I am allowed a maximum of two (2) probation appeals during my academic career at Simmons. To file an appeal, I must be able to achieve good academic standing within three (3) semesters. I must complete (and adhere to) a Measurable Success Plan (MSP), developed in partnership with the Undergraduate Advising Office for the semester(s) during which I am on probation. I must submit a MSP Log (MSPL) at the end of each probation period so that my MSP can be reviewed and my adherence to the agreed upon requirements confirmed. If I fail to submit my MSPL and/or if I fail to meet the goals outlined in my MSP and I have exhausted my probation appeals, I will no longer be eligible for financial aid and will remain ineligible until I achieve good academic standing. (Cum 2.0 GPA and 67% completion rate) If I am on probation and take a leave of absence, it is my responsibility to notify both the Undergraduate Advising Office and the Financial Aid Office in order to update my MSP. Taking a leave and not updating my MSP will be considered a failure to adhere to the terms of my MSP and I will lose my financial aid. If my appeal is denied, or if I miss the deadline, I am fully responsible for all charges on my Simmons account/bill. That by signing this form, I certify all the information, statements, and documentation submitted in this appeal are accurate and true. The decision of the Appeal Committee is final.
